This form is designed to assess performance against the company’s standards and values. The aim is to provide a fair, transparent and consistent process for awarding monthly bonuses.

Note bonus is for persons who ‘Go the Extra Mile.’
It is discretionary so not up for debate but you are welcome to ask for one round of feedback.
Bonus Adjustment Guidelines
The adjustments below are guidelines only. Bonuse’ may increase, reduce or remove bonus based on severity, repetition and overall circumstances.
Dog Welfare & Safety
GPS Forgotten / Not Charged
1st offence: Minus 1 week's bonus
2nd offence: Minus 4 weeks' bonus
Failure to Follow Notes or Safety Procedures
Examples:
Not reading notes
Ignoring instructions
Poor dog management
Preventable incidents
Adjustment:
Minus 1 day to 4 weeks' bonus depending on severity.
Failure to Report Safety Concerns
Examples:
Dog pinning another dog
Escalating behavioural issues
Near misses
Adjustment:
Minus 1 day to 4 weeks' bonus depending on severity.
Attendance & Reliability
Late Arrival
1st offence: Informal warning
2nd offence: Minus 1 day's bonus
3rd offence: Minus 2 weeks' bonus
Continued pattern: Up to full bonus removal
Poor Planning / Lack of Preparation
Examples:
Not reviewing schedules
Raising known issues at the last minute
Arriving unprepared
Adjustment:
Minus 1 day to 2 weeks' bonus.
Time Off clash
Where leave is booked despite another team member already being away and causes operational difficulties.
Adjustment:
Minimum deduction equal to the overlap period.
Maximum deduction of 4 weeks' bonus.
Client Service
Client Complaint
Minor complaint: Minus 1 day to 1 week
Significant complaint: Minus 1 to 4 weeks
Serious complaint causing loss of client or reputational damage: Up to full bonus removal
Time taken to deal with it taken into consideration.
Poor Client Communication

Failure to provide updates
Adjustment:
Minus 1 day to 2 weeks' bonus.
Teamwork & Communication
Complaint from Team Member
Where concerns are found to be genuine and relate to communication, professionalism or teamwork.
Adjustment:
First issue: Minus 1 day to 1 week
Repeated issues: Minus 1 to 4 weeks
Serious misconduct: Up to full bonus removal

Repeated issues will generally result in larger deductions than first-time occurrences.
Where a team member openly acknowledges an issue, takes responsibility and demonstrates clear steps to prevent a recurrence, any deduction may be reduced or waived. They may be re taken into account in a future month if they do in fact re occur.
Remember the pay charged to clients for you is your base pay. If you do not get bonus its not because you’ve done a bad job its because you’ve done a normal job. The bonus is for going beyond that. A safe way to get it it might be to think what have you done each day to earn bonus for that day.
This form is not compulsory but gives you an opportunity to let me know about stuff I may not have seen that you think might earn you bonus. Also if you are aware of stuff that might affect bonus you can acknowledge it here and show how you’ve learnt from it/ made up for it and then this could help avoid loss of some bonus. However if similar things happen in the future then that thing will be re taken into account.
